Please sign at the bottom of the form, including your relationship and address. We ask when faxing this form back that you either have your signature notarized or send an enlarged photo copy of your driver’s license or other photo ID so that we have verification of the signer. Step-by-step instructions for completing the form can be found on page 7 of the form itself.
